To qualify, complete the Vendor Questionnaire and New Vendor Set Up Form and return along with the following additional information.
You may also provide any additional information relative to assisting our evaluation of your company, (e.g., - industrial references, contract person and phone number).
| 1. Certificate of insurance | (CPSG requires $2 million - General and Auto liability and/or Excess Liability and Workers Compensation coverage). Certificate must name CPSG as an additional insured. |
| 2. Drug program | Program must include screening for:
a} new hires (pre-employment)
b} testing for cause
c} random testing |
| 3. Safety program | Contractor's safety manual and procedures |
